Vaginismus is a penetration disorder whereby women experience persistent difficulty with achieving vaginal penetration. It is often characterized by the involuntary contraction of the pelvic floor muscles in response to physical contact or pressure which prevents non-painful sexual intercourse and may cause the vaginal entrance to appear smaller. 

The incidence of vaginismus is varied with research reporting as low as 5% and up to 20% (Spector & Carey 1990). However, these stats may be conservative due to fear or embarrassment in expressing concerns or challenges.

 

 

WHAT TYPES OF VAGINISMUS EXIST?

Vaginismus is typically classed as either primary or secondary.

  • Primary Vaginismus refers to the condition when a female has never had comfortable penetration and, in some cases, has never achieved penetrative intercourse which can have implications for the consummation of marriage and fertility.

  • Secondary Vaginismus occurs after a period of normal sexual relations prior to the onset of symptoms. For example, this may occur after a traumatic event such as abuse or having a baby.

Whilst there is a subset of women whose symptoms stem from physical or psychological abuse or a conservative upbringing, for some women the underlying cause may be a reaction to medical conditions such as Endometriosis, hormonal fluctuations, UTIs, recurrent Thrush or Constipation. And in some women, there may be no identifiable cause whatsoever.

HOW CAN A WOMEN’S HEALTH PHYSIOTHERAPIST HELP MY VAGINISMUS SYMPTOMS?

The first step in helping women find relief (YES, there can be a resolution of symptoms!) is to run through a detailed history. Some questions I tend to ask my patients include:

1.     Have you ever been able to successfully use tampons?

2.     Have you ever had sexual intercourse or penetration of any kind?

3.     Have you had a history of abuse (sexual, emotional, physical) that you are aware of?

4.     Have you had recurrent yeast infections or UTIs?

5.     Do you have a history of painful or difficult periods?

6.    Are you fearful of sex?

After this, I often talk about ladies through the Vaginismus cycle of pain.

 

 

SHOULD I PERFORM KEGELS OR PELVIC FLOOR CONTRACTIONS?

You can perform the following exercises. 

1. Belly Breathing

Abdominal Breathing - Vikasa

By practicing deep conscious belly breathing, as opposed to chest breathing, you will activate the parasympathetic nervous system and relax the lower abdominal muscles, which connect into the pelvic floor. Lie down on your back, place your hands on your belly and as you inhale let the belly rise up like a balloon, and as you exhale, the belly drops back down towards the spine. Do at least ten breaths like this, three times a day. Another tip is to notice during the day when you are a) holding your breath and b) sucking in your tummy. Let your tummy go, relax the abdominal muscles, and take another deep breath. This takes presence and awareness during the day (not just during your yoga practice).

We often hold a lot of tension in our inner thighs, which extends into our pelvic floor. When we stretch, relax, and open the groin muscles, naturally, our pelvic floor will start to relax and soften. This Yin Yoga stretch can be done when you're watching TV or sitting on the floor.

Simply bring the soles of your feet to touch, knees splayed out wide, heels about one foot in front of your groin (or further depending on your flexibility), and then gently fold over the feet. Hold for 3 - 5 minutes and practice at least once a day, incorporate deep belly breaths and consciously relax your yoni muscles as you hold the stretch.

 



3. See a Pelvic Floor Physio.

If your Vaginismus is chronic, you may need to go to a local pelvic floor physiotherapist. Your physio will insert her fingers (with consent) inside your vagina and massage the muscles, just like they would an aching neck, shoulder, or any other sore muscle in the body.

The vagina is simply a muscle that may need some therapeutic massage to help release tension (makes sense!). The physio should teach you how to correctly activate your pelvic floor muscles (Kegels exercises) and they may also suggest a set of dilators to help relax and stretch the muscles. We suggest trying “The Rosie'“ for Rose Quartz crystal dilating.

 

 

SHOULD I SEEK ADVICE FROM OTHER HEALTH PRACTITIONERS?

It takes a village…

A team approach is the best approach. Working alongside your doctor we help identify the best course of treatment for you. I encourage women to seek the support of a psychologist or sex therapist to help address any fear or anxiety.

Where possible, I highly encourage ladies to discuss and be open to their partners about their experience and journey.

And, I do stress that it is a journey, not a sprint.
